Who needs a Form CM-010?
Form CM-010 or the Civil Case Cover Sheet should be filed with the Superior Court of California by any of the parties to a particular suite claiming for recovery of not more than $25,000.
What is Form CM-010 for?
This Cover Sheet indicates the case type and whether the case is complex (large number of separately represented parties, witnesses, evidences, etc.) or not. This information will be used to compile statistics about the types and numbers of cases filed. The cover sheet must be filed only with the initial paper. Failure to file a cover sheet with the first paper filed in a California civil case may subject a party, its counsel, or both to sanctions of the California Rules of Court.
